The digestive endoscopy unit was built in 2004 and because of its unique conception and technical equipment it is one of the most modern in the Czech Republic. Almost eight thousand endoscopy procedures are performed there every year. The endoscopy unit provides all basic methods of digestive endoscopy (gastroscopy, colonoscopy, enteroscopy and ERCP) and all therapeutic methods (bleeding control, polypectomy, endoscopic mucosal resection, drainage procedures, dilations and stent insertions). It also provides abdominal ultrasound (including both targeted and untargeted biopsies of the liver and biopsies of focal abdominal masses), linear and radial endoscopic ultrasonography and choledochoscopy are performed in this unit. The endoscopy unit also provides complex therapy for choledocholithiasis‚ mechanical and electrohydraulic lithotripsy; ESWL in cooperation with Urological Department), percutaneous endoscopic gastrostomies (PEG) are introduced and endoscopic treatment of haemorrhoids is performed. Complex diagnosis and therapy of small bowel diseases is one of the main programmes. All endoscopic examinations of the small bowel are performed: push-enteroscopy, intraoperative endoscopy (in cooperation with the Surgical Department), capsule endoscopy and double balloon endoscopy (as the first Czech department).
The digestive endoscopy unit has all modern diagnostic technologies, such as zoom and high resolution endoscopes and devices with electronic image processing using optical filters (NBI - narrow band imaging, FICE - Fuji Intelligent Colour Enhancement). Confocal laser endomicroscopy was set into operation at the beginning of 2007. This unique diagnostic device is the only one of its kind in the Central Europe.
The endoscopy unit is a nationwide pioneer in “Standards of safe digestive endoscopy” (an analogy to good clinical practice).
Portal hypertension is the main clinical and scientific programme of hepatology. TIPS (transjugular portosystemic shunt) introduction and complex subsequent care is provided in cooperation with the Radiological Department. This programme is unique in the Czech Republic not only in terms of its duration (the first TIPS in the Czech Republic was inserted by Prof. Krajina and Prof. Hůlek as far back as 1992) but also in terms of the most extensive experiences (a series of more than 670 patients at the beginning of 2007 being one of the largest in Europe).
In terms of non-invasive gastrointestinal diagnostics pH metry, electrogastrography, esophageal and anal manometry, anorectal biofeedback, function liver tests (especially utilising indocyanin green) and indirect energometry (calorimetry) are performed. The specialised outpatient clinic also provides complete home enteral nutrition (for Crohn´s disease patients in particular). Breath testing is also an important part of functional diagnostic. The department performs both breath tests using natural non-radioactive carbon isotopes and breath tests based on hydrogen and methane exhaled breath analysis (both in cooperation with the Institute of Clinical Biochemistry and Diagnostics).
